JANUARY CASE OF THE MONTH Eye Enucleation
The Animal Hospital of Cornelius case of the month was recently performed for a toy poodle patient who had severely damaged her left eye going under a fence. The surgical procedure performed is called Eye Enucleation. During this procedure, the eyeball is removed without removing all of the surrounding tissue from the eye socket. After the eyeball is removed, the eyelid edges are permanently sutured closed.
Izzy is a 5.25 lbs. one year old female toy poodle. Izzy's owner presented her to the animal hospital after discovering Izzy partially under the fence in their yard. Izzy had injured her left eye. It was partially out of it's orbital socket. Dr. Jones discussed that the eye was permanently blinded as a result of the injuries sustained and there was no option for salvaging the eye. It was determined that the enucleation surgical procedure should be performed to alleviate Izzy's pain and reduce the susceptibility of infection spreading from the eye. Blindness and enucleation are tolerated well by dogs and cats. Izzy was admitted and fasted prior to surgery, as directed. Prior to surgery, Izzy was administered a sedative and had an intravenous catheter placed for the administration of fluids and medications. Pre-surgical prep included induction under general anesthesia, with a breathing tube (endotracheal tube) placed to allow delivery of oxygen and gaseous anesthesia. The surgical site was then clipped and cleansed with an anti-septic solution. While under anesthesia Izzy's vital parameters (heart rate, respiratory rate, blood pressure, heart rhythm - EKG, oxygenation) were monitored to ensure her well being. A heated underlayment maintained her temperature during the procedure adding to her comfort. Pain was controlled both during and after surgery with analgesics. After Izzy was prepped for surgery, the eye was removed, surrounding tissue was left and the eyelid skin layers were then sutured together closing the orbital opening to complete the operation.
After surgery, the incision must be protected and Izzy was sent home with an E-collar to prevent rubbing or further trauma to the surgery site. Oral antibiotics were prescribed to limit infection. Izzy responded well to the surgery and has progressed very well toward recovery. Animals like Izzy, with one eye enucleated, do not appear handicapped with respect to their mobility or ability to function, and behavior is usually unaffected. Izzy is still a lively little girl, and stays very active in her household.
